PlayTennisLV Waiver
1. Assumption of Risk
I acknowledge and fully understand that participation in tennis activities involves inherent risks, including but not limited to physical exertion, collisions, falls, contact with equipment, and potential exposure to varying weather conditions. I voluntarily assume all risks associated with participation in this clinic, whether known or unknown.
2. Release of Liability
In consideration of being allowed to participate in the tennis clinic, I hereby release, waive, and discharge the clinic organizers, coaches, volunteers, facility owners, and any affiliated persons or entities from any and all claims, liabilities, demands, or causes of action arising out of or connected with my participation. This includes claims related to injury, illness, property damage, or other loss, even if caused by negligence.
3. Medical Authorization
I certify that I am (or my child is) physically able to participate in the clinic. In the event of injury or medical emergency, I authorize clinic staff to obtain necessary medical treatment. I assume full financial responsibility for any medical services provided.
4. Photo/Video Preference
Photo and video permission is handled as a separate registration choice and is not required for participation. When permission is granted, a parent or guardian may contact PlayTennisLV to revoke future use.
5. Behavior & Safety Expectations
I agree to follow all instructions, rules, and safety guidelines provided by coaches and staff. I understand that failure to comply may result in removal from the clinic without refund.
6. Refund & Weather Policy
I acknowledge that refunds or credits, if offered, are at the discretion of the clinic organizers. I also understand that weather-related modifications, cancellations, or rescheduling may occur.
